From vial to syringe units, step by step
If your medication arrives as a powder in a vial, there is arithmetic between you and your dose that pen users never encounter. It is not complicated arithmetic, but it has three places where a small error produces a large one — and all three are avoidable once you can see the chain.
The chain, in order
Every number depends on the one before it. Get the first wrong and everything downstream is wrong by the same proportion.
Step 1: Concentration
Concentration = amount in the vial ÷ volume of diluent added.
A 10 mg vial reconstituted with 2 mL of bacteriostatic water gives 5 mg/mL. The same 10 mg vial with 1 mL gives 10 mg/mL. Same vial, same total medication, completely different concentration — and therefore a completely different syringe reading for an identical dose.
This is the number to write down. It is the one people most often fail to recall three weeks later, and without it none of the rest can be reconstructed.
Step 2: Draw volume
Draw volume (mL) = your dose ÷ concentration.
A 2.5 mg dose from a 5 mg/mL vial is 0.5 mL. Units have to match: if your dose is in micrograms and your concentration in mg/mL, convert before dividing, or the answer is out by a factor of 1,000.
Step 3: Syringe units
Syringe units = draw volume × the syringe's units-per-mL rating.
This is where the most consequential errors happen, because the rating is a property of the syringe rather than the medication.
- On a U-100 syringe, 1 mL is marked as 100 units. So 0.5 mL reads as 50 units.
- On a U-40 syringe, 1 mL is marked as 40 units. The same 0.5 mL reads as 20 units.
- On an mL-marked syringe, you read the volume directly.
Reading a U-40 syringe as though it were U-100 — or the reverse — produces an error of more than double in one direction or less than half in the other. Check what is printed on the barrel rather than assuming.
Where it actually goes wrong
- Misremembering the diluent volume. By far the most common. Write it on the vial or in a log the moment you reconstitute.
- Reconstituting the next vial differently. If you used 2 mL last time and 1 mL this time, your familiar syringe reading is now wrong. Record concentration per vial, not once.
- Mixing up mg and mcg. A factor of 1,000, and it looks plausible enough to miss.
- Assuming the syringe rating. Check the barrel. Every time you open a new box.
